Create Forms

After Change Settings Page go to Create-form.php

And Add Title , Subtitle and Check the Checkbox Which Column field is Required For Your Forms

After Click The Field go down And Click To Form Create

After Click Your Dynamically Form url is Create And Copy this url

Then Go To QR Code Generator

After saving your next step will be Create QR Code & URLS
